We require this information to assist the allied health professional in supporting you.

We will usually collect your information directly from you. Sometimes, we may need to collect information about you from a third party (such as a relative or another service provider).

How does Lets Reseed PTY LTD  use your information? 

Lets Reseed PTY LTD  will use your personal information for the purpose you have given the information to us. We will use your information to provide therapeutic support and social work services, manage our relationship with you, and contact you concerning matters concerning your care. We may also use your information for other purposes under the Privacy Act 1988.

 

To whom might we disclose your information?

We may disclose your information to the following people:

  • disclosure to other professionals involved in your support and treatment

  • your personal information will generally only be used by the allied health practitioner involved in your care. However, your care may occasionally be provided by several health professionals (for example, a social worker, speech pathologist, occupational therapist, and psychologist) working or consulting together. We may disclose your information to these and other health professionals involved in your care.

  • the referrer

  • Lets Reseed PTY LTD  will usually send a summary to the referrer (i.e. your medical practitioner) following closure from Lets Reseed PTY LTD  or, at other times, as required for your care. 

  • please let us know if you do not wish us to send the referrer a copy of your summary and if the referrer’s details have changed. 

  • relatives, guardians, close friends or legal representative

  • we may provide information about your condition to your parent, child, other relatives, close personal friends, guardians, or a responsible person unless you tell us that you do not wish us to disclose your health information to any such person.

 

Other uses and disclosures:

To provide the best possible environment in which to treat and support you, we may also use or disclose your personal and health information where necessary for:

  • activities such as clinical supervision, quality assurance processes, accreditation, audits, risk and claims management, client satisfaction surveys and staff education and training.

  • invoicing, billing and account management.

  • liaise with your funding source, Medicare or the Department of Veterans Affairs, and, where required, provide information to your funding source, Medicare or the Department of Veterans Affairs, to verify the treatment provided. 

  • to send you standard reminders, for example, for appointments and follow-up care, by text message or email to the number or address you have provided to us.

  • compliance with relevant legislation regarding a client’s actual or perceived harm to self or others or other mandatory reporting requirements. 

 

Other uses with your consent:

We will only disclose personal information by the Privacy Act 1988. This means that personal information may be disclosed:
for the purposes for which we have advised that we are collecting it and for related purposes that the individual would reasonably expect,

  • where we have the consent of the individual to do so,

  • as required by law, or

  • under other circumstances where permitted under the Act.


We may need to disclose some of your personal information to relevant staff during our business activities. With your consent, we may also use your information for other purposes, such as including you on a marketing mailing list or conducting research. Please note, however, that we will not use your information this way unless you give us your express consent. We will not disclose your personal information to any individual outside Australia.

 

Access to and correction of your information 

  • You have a right to access the information we hold about you and to request an amendment to your information if you believe it is inaccurate. 

  • If we do not agree to change your medical or health record/personal information as requested, we will permit you to make a statement of the requested changes and enclose this with your record. 

  • Should you wish to obtain access to or request changes to your health record, you can ask our Privacy Officer (see details below), who can give you more detailed information about Lets Reseed PTY LTD's access and correction procedure.

  • Please note that Lets Reseed PTY LTD  may recover reasonable costs of supplying this information.

Lets Reseed PTY LTD  will take reasonable steps to ensure that the personal information we may collect, use or disclose is accurate, complete and current.
