Please complete this form to refer to Lets Reseed with the relevant consent.

We will contact the client/patient/participant for a free initial consultation. For children, young people, and individuals with guardians or parents, we will involve relevant caregivers in the initial consultation to ensure a comprehensive understanding of their needs. We prioritise accessible and inclusive communication, so if phone or email intake is not suitable, please inform us, and we will arrange an alternative.

If the referral aligns with our services, we will send the client/patient/participant (or their parent or guardian) an intake form to complete. Following the initial consultation and acceptance into our service, we will seek permission to obtain any specialised reports, letters, and documents necessary to support their therapy and support at Lets Reseed.

  • As a registered provider with the NDIS Quality and Safeguards Commission, we are accredited for:

    • 0128 Therapeutic Supports (our primary support service)

    • 0120 Household Tasks (stay tuned for updates)
